Japan | Mandatory submission of “Tuberculosis-Free Certificate” for CoE/ visa applications (updated October 22, 2026)

Summary

Since June 23, 2025, nationals of certain countries seeking mid- to long-term residence in Japan must submit a “Certificate of Non-Tuberculosis” (TB-Free Certificate) with their Certificate of Eligibility (CoE) or visa application. Applicants must first complete “Pre-Entry Tuberculosis Screening” (JPETS) at an overseas medical institution designated by the Japanese government before applying for a CoE. The requirement already covers nationals of the Philippines, Nepal and Vietnam, and will extend to Indonesian nationals from January 2027. (Updates in this version are underlined.)

The requirement is also expected to extend to nationals of Myanmar and China, but implementation dates have not yet been set. Those affected should allow extra time to meet the new medical requirements when planning their move to Japan.

The detail

Details of the JPETS System

Background

Japan has introduced the JPETS due to a rise in tuberculosis (TB) cases among foreign nationals. Nationals from countries with high incidence of TB must provide proof of being TB-free before entering Japan for mid- to long-term stays.

Who is affected

The requirement applies to nationals from the Philippines, Nepal, Vietnam, Indonesia, Myanmar and China, including Digital Nomad visa holders and their dependents newly arriving in Japan as mid- to long- term residents.

Exceptions:

  • Individuals residing outside the eligible countries who hold a valid residence permit or similar documentation, regardless of the length of stay.
  • Existing residents re-entering Japan with valid re-entry permits.
  • Those entering Japan under visa programs that already mandate TB screenings, such as the Specified Skilled Worker visa.

Timeline

*If the CoE was issued based on an application submitted before this date without a TB-Free certificate, the entry visa may still be processed after the implementation date.

JPETS screening process

  • Applicants must complete a medical examination, including a chest X-ray, at an institution designated by the Japanese Government.
  • Applicants must present a passport to verify their identity.
  • The TB-Free Certificate will be issued in English and Japanese using the official Ministry of Health, Labour and Welfare (MHLW) form, with the applicant’s photo and passport details.

For the latest list of the designated medical institutions, visit MHLW’s website: https://jpets.mhlw.go.jp/emi/

Validity of the certificate

  • The TB-Free Certificate states its validity period, as determined by the medical institution.
  • The TB-Free Certificate is generally valid for 180 days from the X-ray date.
  • Validity is shortened to 90 days if: within 2 months before the X-ray, the applicant lived with a family member diagnosed with active TB/ within 2 months before the X-ray, the applicant spent a prolonged period in an enclosed space with someone diagnosed with active TB.
  • Applicants may apply for an entry visa and enter Japan after the TB-Free Certificate expires, provided they submitted the CoE application while it was valid.
  • Applicants should keep a copy of the certificate, as it may be required on arrival or during their stay in Japan.

When is submission mandatory

The TB-Free Certificate (original or copy) must be submitted in these scenarios:

  • During a CoE application in Japan → Failure to submit will result in application rejection.
  • During an entry visa application at a Japanese consulate/embassy, only if: submission during CoE application was unavoidable, OR the applicant is applying directly for a visa without a CoE.
  • Exceptions are determined on a case-by-case basis by immigration authorities.

What this means

For employers:

  • When arranging for foreign nationals to work in Japan, allow for the extra time and cost of the JPETS medical examination and obtaining the TB-Free Certificate.
  • Monitor updates on affected nationalities, residence statuses and implementation dates to avoid onboarding delays.
  • Check well in advance that employees hold valid passports, which are required for the JPETS medical examination.

For individuals:

  • Allow time to complete the JPETS medical examination and obtain the TB-Free Certificate.
  • Have a valid passport ready for your medical examination at a government-designated institution.
  • Retain a copy of your TB-Free Certificate after arriving in Japan, as you may need it.
  • You must submit the certificate with any CoE or visa application made after the implementation date.

Employers and individuals should plan ahead for the new JPETS requirements to keep relocations on schedule.
